Cedar Creek of Los Gatos

The Creeks have specialized in Alzheimer’s and dementia care since 1991. Emphasis is placed on respect for our residents, and acceptance of who they are today, as they continue their life journey. We create an atmosphere of caring and compassion for our residents through every stage of their memory loss.

About Cedar Creek

  • 58 bed, Assisted Living Community for residents with dementia (29 shared rooms)

  • Serving the community since 1996

  • Long term and respite care

  • 1 acre of secure grounds including walking paths, therapeutic gardens and bird aviaries

  • Three daily meals and snacks prepared by our classically trained chef

 

  • Licensed nursing staff

  • Medication management and administration

  • Assistance with activities of daily living (ADL’S)

  • Ratio of residents to caregiver = 7:1

  • Dedicated activity building

  • Specially designed Alzheimer’s activity programs, including music, art and pet therapy

  • 10 hrs. of activity a day!

  • Laundry and housekeeping service

  • 24-hour staffing

  • Beauty salon and barber shop

  • Hospice care

  • Incontinence care

  • All staff training is provided by administrator, nurse, and other professionals depending on specific healthcare topic.

  • Trainings exceed the state requirements of 8 hours dementia specific training per year.  In 2013 all staff are required to attend monthly mandatory staff in-services

  • Must have diagnosis of Alzheimer’s or dementia.

  • Cedar Creek is secured so we can take residents who wander.

  • Safe ambulatory/transfer status. They can “age” to a wheelchair.

  • No type 1 diabetes, daily injections, tube feeding or physically aggressive residents.

  • Immediate response to bed availability

  • Prompt assessments Monday – Friday

  • Assistance in finding alternative placement

  • Private pay

  • Prior to admission, all potential residents will need the following:

  • A Tuberculin test within the last 6 months or chest x-ray within the last year that shows a negative result.  The result must be in writing and submitted to Cedar Creek prior to admission.

  • A Physician’s Report (LIC 602) completed and signed by the responsible physician, received by Cedar Creek prior to admission.

  • A pre-admission assessment of the prospective resident by the administrator or her designee.
